This is an interesting question. I have technically used my own name. Well, a variation on my last name, kind of like George Lucas with Luke, because this character used to be more autobio and then became less so over the years. But I kept the name anyway because I like it?! One character actually has the same first name as my one cousin's middle name, just because I liked it (of course). I also not so subtly named a villain character after a former(?) friend. Granted, they are nothing alike outside of gender, and the villain is fairly complex, just like my feelings towards that friend. That story may never get published, but who knows... I really don't think that person would notice or care.
I would consider using first/last names of family members or close friends, but they would likely be for background characters that have absolutely zilch to do with the real person, or I'd alter the spellings slightly. Apparently my uncle did that very thing for a book he wrote and had published. I have yet to read it though. Haha, but don't ask if my name was used because idk!
