I'm an artist so it's a little different because I am also an illustrator as well as a comic artist. For me lower tiers get 1 post a week regularly (usually work in progresses and high res non watermarked finished illustrations, usually not about my comic directly) higher tiers get pages 1 month early (just because that's how my schedule worked out) and the highest tier gets monthly merch at a discount plus all the rest. All patrons get their names in the thank you message at the end of each part.
I mainly made my patreon because there are some scenes in future chapters that will be heavily censored for webtoon and tapas and I wanted to have a well established backlog and rhythm by the time I get there, so anyone already on there is a bonus.
As for the individual posts getting confusing, you can sort posts into folders by tagging them, so you can tag each project with the project's name so if people only want one they can just peruse the tag. As long as you label and tag your posts clearly (like post "Project X - chapter 1" in the "Project X" tag) it should be pretty easy to navigate.
As for how you link your patreon to the other sites, webtoon actually has a specific plugin for patreon where you just drop the link in the box for it in your comic menu, it'll automatically offer it to readers at the end of the episode. On tapas it's customary to just pop the link in the episode description.
