otherwise there is the basic places like cafe press and zazzle. but they don't actually make charms with shapes, they do more basic shapes I think. printing books possible lulu
if you're making them on your own. I used to print mine out from computer, use construction paper for the backs, laminating pouches and then use some little 'key' charms from arts and craft stores to connect them. but those are just simply things.
I don't know how other people make the really detailed ones, all I can think is it's custom made at certain places. not sure if that helps but maybe someone else knows more, I don't really make things by hand anymore I don't have time. ^-^
