I sketch and ink with traditional mediums, and color "screentone" digitally.
Primary tools:
.9 mm HB led for most sketching
.7 mm HB finer details
Erasers (large one, gummy, & stick eraser)
I used to use Canson marker paper, but recently switched to Strathmore Bristol so I could Copic color when I want (heavier paper). I get large sheets and cut them in half to double the page number 
Zig Kuratake Black in 60 (with synthetic glue)
Saji nib/fine point brushes/technical pens mostly Microns, but some Faber Castille, too.
I sometimes will use other nibs, but that's my primary one.
I also use stencils for measuring heads and body height, as well as rulers and such/Dad's old drafting brush.
Program:
Right now, unless I want to have some fun with Copics/colored inks/etc, I will color and "screentone" in Photoshop CS5.
I'm not happy with my screentones. I use the same ones I downloaded from some manga pack years ago, but I also haven't had the gumption to force myself to learn Manga Studio (this is the original version of Clip Studio. It functions! But I dunno how to use it XD I need to commit time).
Were I rich, I'd like to use screentone/zipatone sheets more often, but they just are too expensive.
It's cool seeing all the programs/mediums people use for their comics :3
