Actually, in the comics, Wolverine has died several times and, more recently, died "permanently" by being completely encased in adamantium. That was like a year or so ago, though, and with Disney buying out Fox, he'll probably be revived soon enough if he hasn't been already x).
As for a character who never lived, Shalltear Bloodfallen from Overlord; she has been Undead since her creation =P.
