We ghostwrite novels and non-fiction books for people time to time, and I'll never personally forget this absolutely hilarious gaff from someone's book I was editing:
Back at the bridge, a Massachusetts State trooper crouched as he took a pole and pulled the small, lifeless and bedraggled body of a little redheaded girl to shore. As he lifted her body from the water, her head wobbled freely, the neck having been snapped.
Art Freewell was finally realizing his long anticipated dream. He lowered his fishing line into the water and sat down in his canoe.
So...yeah, from the context, how I read, I suddenly stopped editing and had a good laugh for about 5-10 minutes....
