I found using flashcards at first helped. There are a couple of programs that you can do mind maps on and that's good as well. Start with the 'main' branch and then add on to it.

Yes - just tag it so your readers know it's mature. The rules are a little different comics cuz of images and stuff, but novels...you can get away with some really mature stuff.
I think it's all about what feels right for the story. Some romance novels have only one or two mature scenes, others have one every other chapter. Do what feels right.
