If he's a criminal, he's more likely to know before she does - the thing about criminal empires is that they often have an idea of what the families of their members are gonna do well before those people do.
If a boss, say his name is Todd, has a worker named Phil, and he knows that Phil is married to a woman named Elsa, that's important info. Todd needs to know if he can trust Phil, and to do that, he needs to know how Phil treats Elsa, whether Phil trusts Elsa, and how much Elsa knows. Who does Elsa talk to? To manage secrets, you must know intimately the spiderweb of the people you're connected to. One false word from the wrong person, even on accident, and... bada bing, bada boom. You're dead.
Someone tipped the cops after they heard Elsa say Phil was gonna be down in the cement pits of the construction site, because THAT person knows that Phil works at the aquarium but their recently missing brother DOES work that particular site.
You get the drift?
So, for Phil to NOT know that Elsa is pregnant, for NINE months, TWICE? It's not likely, because Phil needs to tell Todd how Elsa is doing. That's part of keeping up with the family, knowing how your workers lives are going. That's to make sure nobody hides debt, or drug problems, etc, which is likely to have them play turncoat for the cops and sell the crew out. Not to mention, criminals have their ways of getting information... like using a pregnancy test on Elsa's piss after she forgets to flush, because he's noticed she's eating differently. Todd hears that, he knows exactly what to check for: "Do ya think she's preggo, Phil? You should check. Real covertly, don't scare the broad."
Now, Elsa knows her own body just as well, no saying she doesn't have agency in this situation. But the fact of the matter is... if she's pregnant, then she knows it, Phil knows it, and Todd knows it. And just as likely, the whole gang knows it. You gotta stay tight-knit if you wanna survive in the slums.
In fact, if your character lies to her man about those two kids, he probably already knows, and is lying about that, too.
